Structure, Milieu, and Timing: A Supramolecular Perspective on Sarcoidosis and Integrative Medicine

Biography:

Dr. Hamid R. Ansarian is a practicing family physician in Manitoba, Canada, and a graduate of the University of Manitoba. Alongside clinical practice, he has pursued research spanning basic sciences, disease pathophysiology, epidemiology, and artificial intelligence in medicine. His work reflects a broad interdisciplinary interest in understanding life and health to improve patient care. Dr. Ansarian has explored concepts extending from physics and chemistry to psychology and philosophy and has co-authored scientific publications in diverse fields ranging from field epidemiology to nuclear magnetic resonance. His current interests focus on integrating scientific principles to advance medical understanding and healthcare.

Research Interests:

Sarcoidosis remains a challenging inflammatory disease in which biologically active therapies often improve symptoms without producing complete structural remission. This presentation proposes a supramolecular framework in which disease progression represents a hierarchy of self-organizing events occurring at different levels of biological organization. In this model, molecular recognition (SDA?) generates an inflammatory milieu (SDA?), which supports the formation of granuloma architecture (SDA?), followed by tissue remodeling and fibrosis (SDA?). Using recent findings from the phase III EFZO-FIT trial of Efzofitimod as an illustrative example, the presentation argues that improvement in patient-reported outcomes despite failure to achieve the primary endpoint may reflect a mismatch between the level targeted by therapy and the level at which disease has become established. Efzofitimod acts primarily on the inflammatory environment and myeloid signaling pathways, whereas persistent granuloma organization may exhibit increasing structural stability and reduced dependence on the milieu that originally promoted its formation. This perspective suggests that timing and level of intervention are critical determinants of therapeutic success. The framework also provides a mechanistic basis for several principles long emphasized in traditional and integrative medicine, including early intervention, restoration of balance, and the use of multi-component therapeutic approaches. By integrating supramolecular chemistry, immunology, and concepts relevant to traditional medicine, this work proposes that future therapies may need to address both the inflammatory milieu and the structural organization of disease, thereby advancing a multi-level approach to medicine.
